BCHC’s goal is to help you access healthcare and maintain wellness regardless of your ability to pay. We provide services on a Sliding Fee Scale for those patients who qualify. Patients must submit financial documentation annually to be eligible. If you have financial concerns about receiving services, please ask to see the BCHC Eligibility Specialist.
Documentation required at the time of service for Uninsured Patients to qualify for the Sliding Fee Scale:
- Photo ID (Driver's License/ID Card/Passport)
- Proof of Household Income (Pay Stub/SSI Disability/Financial Aid/EBT (Food Stamps) Award Letter, etc.)
- Proof of Residence (Driver's License/Utility Bill/ Lease Agreement/Voter's Registration Card, etc.)

Please complete the forms as soon as possible. We can start preparing for your visit to make your visit as smooth as possible.

Preparing for your Visit
We want to ensure that you are prepared for your visits, please utilize our appointment checklist below as a reference guide for your visit. This will allow us to meet all of your needs in a timely fashion. Same-Day Appointments are available to established patients for urgent and walk-in appointments. Routine appointments can be scheduled within 2 weeks when necessary. New patient appointments are available daily
Appointment Checklist
Gather documents to bring to your doctor’s visit:
Adult New Patients:
- Photo ID, Insurance Card, Social Security Card
- Any laboratory tests results, x-rays or EKG’s
- A current list of your medications and dosages, including over the counter medications/vitamins
Pediatric New Patients
- Photo ID of parent/legal guardian
- Legal guardianship documentation is required
- Insurance Card
- Birth Certificate
- Social Security Card
- Patient and Parent/Legal Guardian
- Any laboratory tests results, x-rays or EKG’s
- A current list of your medications and dosages, including over the counter medications/vitamins

Requesting Medical Records
A written authorization form must be submitted to the Health Information Management Department. The form can be filled out at our BCHC location or mailed. Patients requesting copies of their own records will take no longer than 15 days to process. All other requests will take no more than 30 days to process. Please use the contact information below.
