Monday through Thursday: 8 AM to 5 PM
Friday: 8 AM to 1 PM
All patients are seen by scheduled appointments and for acute needs. Walk-ins are accepted based on availability.
On your first visit, you will need the following forms filled out prior to your appointment:
- Registration Forms
- Health Questionnaire
- Medical Record Release Authorization
- Insurance Information
When you arrive for your first appointment, please bring the following:
- Photo identification
- Insurance card
- Social Security card
- A list of all your medication that you’re currently taking, including vitamins and over the counter medications.

Bond hosts Philips and MagCorp
We are thrilled to share that an agreement has been reached between Philips, a global health technology company, and MagCorp, a Tallahassee-based startup. This agreement will connect scientists from the Florida State University-headquartered National High Magnetic Field Laboratory with industry professionals to make MRI imaging more affordable, accessible, and sustainable, across the world.
A few weeks ago, we had the opportunity to host executives from Philips to discuss Bond’s mission as a Federally Qualified Health Center and how their research could impact the quality of care we provide to patients every day.
Another great story by the Tallahassee Democrat and ace reporter Nada Hassanein about the latest joint community effort between Apalachee Center, Inc. and Bond Community Health Center! As always, it's a pleasure to live in a community with such a culture of collaborative partnership in the healthcare arena. Bond Community Health Center, Inc. (Bond CHC; the Center) is a 501(c)(3) community health center deemed as a Federally Qualified Health Center for greater than 30 years. Bond provides a patient-centered approach to quality primary and preventive healthcare services for residents of Leon, Gadsden, Wakulla, Jefferson, Taylor, Franklin, Liberty and Madison counties.
Smile Connection
We provide comprehensive dental care for children through age 18 including exams, cleanings, x-rays, and fillings. We also provide dental care for expectant mothers and diabetic patients of the clinic, which includes nutritional counseling and hygiene services. Urgent care dental services including palliative care for the relief of pain and extractions are available to adults in the community.
