Our services include, but not limited to Adult/Primary care; Geriatric; Pediatric; Outreach, and Transportation. We are located in the heart of the Springfield community in Tallahassee, Fl. Our goal is to provide access to quality and comprehensive healthcare for the community and surrounding area. It is our responsibility to make sure that we provide the best health care possible to our patients. We are a fulltime healthcare facility that specializes in the care of patients of all ages.

Hours of Operation
Our normal office hours are:
Monday, Tuesday and Thursday 8:00 AM to 5:00 PM
All patients are seen by scheduled appointments and for acute needs walk-ins are accepted based on availability. Walk-in hours are Monday, Tuesday and Thursday from 8:00 AM until 11:00 AM and resume 1:30 PM until 3:30 PM.
On your first visit, please notify your health insurance company of your new health care provider if required. You will also need the following four forms filled out. Please indicate which doctor you are requesting for your primary health care physician and please mail these papers a week prior to your appointment:

- Health Questionnaire
- Medical Record Release Authorization
- Insurance Information
When you arrive for your first appointment, please bring the following:
- Photo identification
- Insurance card
- Social Security card
- A list of all your medication that you’re currently taking, including vitamins and over the counter medications.

Healthcare for Men
BHAPCC focus on prevention and early detection of common health issues such as diabetes and heart disease. Our services include:

- Screening and treatment for Sexually Transmitted diseases

- X-rays
- Smoking Cessation
- Social Services
Healthcare for Women
Our health care professionals are specially trained to be sensitive to the needs of women and focus on prevention and early detection of common health issues such as breast and cervical cancer, diabetes and heart disease. Women health services include, but are not limited to:
- Physical exams
- Breast exams
- Pap smears
- Screening and treatment for sexually transmitted diseases
- Blood glucose
- Cholesterol testing
Referrals are provided for OB/GYN services, mammograms, ultrasounds, dental, colonoscopy, cardiology, chiropractor, podiatry and ophthalmology, X-rays, and social services etc,
Geriatric Services
BHAPPC geriatric services focus on adults ages 65 and above. Our healthcare professionals focus on accessing the direct needs of the geriatric patients. Geriatric services include:
- Physical exams
- Blood glucose testing
- Cholesterol testing
- Risk assessments.
Geriatric Assessment
The geriatric assessment is performed by a team of professionals specializing in the care of older adults ranging from 65 and above. The risk assessment is performed to access the individual’s physical and physiologic needs and to develop a plan to address their needs.
Pediatric Services
BHAPPC is dedicated to developing advances in pediatric care, promoting the well-being of children and families and serving as advocates for children’s health related issues. Our services include:
- Well Child Exams
- Lead screenings
- Vaccinations
- Social services
- Case management
Outreach Services
Our main goal is to ensure patients have access to quality and comprehensive healthcare by providing outreach services. Our outreach services are used as resources in the community which include:
- Community partnership
- Health fair screenings
- Health education
- Health information
- Referral services
- HIV Health Education/Testing at Community Events.
Transportation Services
Bond Community Health Center focuses on eliminating the barriers to accessing healthcare by providing transportation services to and from the healthcare facility. Patients in need of transportation should call (850) 391-4799 and schedule transportation prior to their appointment.
Patients are asked to maintain an active telephone number so pick-ups may be confirmed. If patients need to cancel transportation please call (850) 391-4799 in a timely manner.
